What Is Interproximal Reduction in Dentistry?

If your dentist recommends shaving down a little bit of your tooth enamel before starting clear aligner treatment, you might have questions. Does it hurt? Will your teeth look different? And why is it necessary in the first place?

Don’t panic! The procedure is called interproximal reduction (IPR), and it’s a common part of orthodontic treatment. While the idea of removing enamel might sound intimidating, IPR is a carefully planned procedure that can help create the space your teeth need to move into better alignment.

Interproximal Reduction Simplified

Interproximal reduction is a dental procedure that involves removing tiny amounts of enamel from the sides of neighboring teeth. You might also hear it called tooth slenderizing, enamel stripping, or reproximation.

The goal is to create a little extra room between teeth so they can move into their intended positions during orthodontic treatment. This can be especially helpful for patients with crowded teeth or small discrepancies in tooth size.

Your dentist won’t remove large portions of your teeth. Instead, IPR involves carefully removing fractions of a millimeter of enamel from specific areas. The changes are typically so small that you won’t notice a difference in the appearance of your smile.

Why Do I Need IPR Before Clear Aligners?

Clear aligners work by applying gentle, controlled pressure to gradually move your teeth. However, if your teeth are crowded or overlap, there might not be enough room for them to move into their ideal positions.

IPR helps address this problem by creating additional space without necessarily requiring tooth extractions or expanding your dental arches beyond what’s appropriate.

Your dentist might recommend IPR to:

  • Relieve mild to moderate dental crowding.
  • Help overlapping teeth move into better alignment.
  • Correct differences in the sizes of neighboring teeth.
  • Improve the shape and contact between certain teeth.
  • Support a more balanced bite and final orthodontic result.

Not everyone needs interproximal reduction. Your dentist will evaluate your teeth and develop a treatment plan based on your individual alignment needs.

What Happens During the Procedure?

IPR is usually quick and straightforward. Your dentist will use specialized dental instruments, such as thin abrasive strips or small rotating discs, to gently remove a controlled amount of enamel between selected teeth.

The process generally involves a few steps:

  1. Your dentist identifies which teeth need additional space based on your orthodontic treatment plan.
  2. A specialized strip or instrument is used to carefully reshape the sides of those teeth.
  3. The treated surfaces are smoothed and polished to maintain comfortable contact between neighboring teeth.
  4. Your dentist may apply fluoride to support enamel health.

Depending on your treatment plan, IPR might happen during one appointment or be spread across multiple visits.

Does Interproximal Reduction Hurt?

Here’s some reassuring news: IPR is generally painless! Tooth enamel doesn’t contain nerves, so removing a small amount of it shouldn’t cause discomfort. Most patients don’t even need local anesthesia.

You might notice some pressure, vibration, or an unusual sensation during the procedure. Some people also experience temporary sensitivity afterward, particularly when eating or drinking something hot or cold.

Because IPR permanently removes a small amount of enamel, it’s important that the procedure is carefully planned and performed by a qualified dental professional. Removing too much enamel can increase the risk of sensitivity and other dental complications. Your dentist will determine how much reduction is appropriate for your teeth.

Is Interproximal Reduction Necessary?

Not always! Some patients have enough room for their teeth to move without additional space being created. Others may benefit from IPR as a conservative alternative to more extensive orthodontic procedures.

If your dentist recommends it, don’t hesitate to ask why it’s included in your treatment plan, which teeth will be treated, and how much enamel will be removed. Understanding the reasoning can make the entire orthodontic process feel less intimidating.

A little extra space can make a big difference in creating a healthier, more balanced smile!
